Breastfeeding Basics: Send your scores

You can request that your evaluation scores be sent by E-mail by submitting this form. By default, the scores are sent to the instructor of the course that you are enrolled in, but you can enter an E-mail address below to send your scores to someone else (eg: yourself).

Notes:

This also works if you are enrolled for CME credit (don't enter an E-mail address below).

Breastfeeding Basics is designed to send scores to instructors automatically, so submitting this form is not usually necessary. It is here in case something goes wrong and there is a need to resend your scores.
