A breastfed infant with poor weight gain can be defined as:

  1. An infant with weight loss continuing after ten days of age, or

  2. An infant who has not regained birth weight by two weeks of age, or

  3. An infant with weight gain below the tenth percentile beyond one month of age, or

  4. An infant with a maximum weight loss of > 10 to 12% of their birth weight.

In a study of 686 Italian newborns, who were solely breastfed according the Baby Friendly Hospital guidelines, 53 (7.7%) had a weight loss 10% or greater in the first 3-4 days of life. Maximum weight loss occurred on day 3-4 for infants born vaginally and on day 4-5 in infants who were born by caesarean section ( Memorize Manganaro, 2001 ).
